POSITION: Job Description

PURPOSE:

The CCBHC is comprised of several community-based programs providing direct care to over 2,500 clients annually, both on site and in the community. Our staff use a culturally competent, person centered approach to help individuals and their families develop skills and resources to improve overall functioning, to instill hope, and to strengthen resiliency. Our programs work closely with community partners to address health disparities in our neighborhoods while also celebrating the strengths and resilience of our communities.

Position Overview:

The Peer Specialist will provide inpatient-to-community transition care, in-home crisis stabilization/treatment services/supports. The peer will support client in linkage to care and engagement with identified onsite providers and community providers and ensure that client support networks are sufficient to meet their needs.

Responsibilities include but not limited to:

Individual sessions with participants may include helping them deal with issues associated with (but not limited to) substance abuse, mental health treatment, family and social relationships; stress and symptom management; activities of daily living; medication management; and independent living or supported housing

  • Provide emotional and practical support to participants as they engage in behavioral health treatment
  • Advocate with and for program participants to achieve personal goals and address barriers such as fear, stigma and communication disparities with community providers and supports.
  • Attend and participate in team meetings and supervisory sessions
  • Monitor and record participant's progress and modify plan according to needs and preference
  • Comply with all required in-service training and staff development
  • Escort consumers to scheduled appointments as needed
  • Demonstrate excellent organizational and time management

Essential Knowledge skills and abilities:

Demonstrate ability to effectively use one's personal story and role model recovery in a way that promotes personal growth and empowerment

  • Knowledge of behavioral health systems, mental health legislation and regulation, and health conditions
  • Ability to manage multiple projects and ask for help when needed
  • Ability to participate on a team to accomplish task
  • Serve as a role model to staff and stakeholders
  • Ability to work with all stakeholders: participants, network and families in a caring and respectful manner, and with due understanding of and consideration for cultural differences
  • Knowledge of treatment, rehabilitation, and community support programs as they relate to members and their families
  • Knowledge of techniques for identifying risk, including crisis management techniques

Requirements:

This position is located in the Midwood Section of Brooklyn, with required travel throughout the borough.

  • H.S. Diploma/ GED
  • New York Peer Specialist Certification through OMH
  • Have at least one year experience in direct services working with persons who have been given a psychiatric diagnosis and/or trauma survivors

COMPUTER SKILLS REQUIRED:

Prior experience with electronic health records and Microsoft Office

VISUAL AND MANUAL DEXTERITY:

  • The candidate should be able to read paper and electronic documents and perform significant data entry into various computer programs
  • Manual dexterity and hand-eye coordination to travel independently using public transportation

WORK ENVIRONMENT / PHYSICAL EFFORT:

  • The majority of time is spent in the field providing services to clients in their homes and other community settings, with less time spent in office-based work.
  • While the offices of the Jewish Board are accessible in accordance with the ADA, the sites to which staff may need to travel may or may not be.
  • To perform the essential functions of this job the candidate must be able to travel within New York City carrying equipment such as a notebook, forms, laptop, mobile hotspot and cell phone weighing up to approximately 10 pounds.
  • To perform the essential functions of this job, the candidate is routinely required to sit (60% of the time) and stand (20% of the time), and travel to and from appointments using public and private transportation options (20% of the time).
  • Risks/hazards associated with the position are those which may be encountered traveling around New York City.
