Under the general direction of the Enterprise Chart Correction Manager, this position is responsible for end-to-end completion of corrections across multiple systems and within the paper record, if applicable.

Accurate management of patient clinical data integrity in the Mass General Brigham Electronic Health Record (EHR) is foundational to the care of all of the patients within the Mass General Brigham network. The Mass General Brigham patient database contains over 11 million patient records and serves as the foundation of care, patient communication and clinical coordination within the network and beyond.

Accurate information in the EHR is fundamental to excellent clinical care. Therefore, as part of the Enterprise Health Information Management department, the Chart Correction Unit is responsible for ensuring an accurate and complete clinical record within the Mass General Brigham system. The Chart Correction Unit directs clinicians, providers and staff on how to make appropriate corrections or amendments within the EHR. The team also has the authority to correct information administratively on behalf of providers, as appropriate. The team works closely with the Information Systems to determine the appropriate correction path to maintain compliance with all regulatory requirements.

The Chart Correction Specialist I is a subject matter expert on chart corrections for Mass General Brigham. In this role, the Chart Correction Specialist I collaborates with providers, clinicians, and multiple Partners IS teams to provide critical and complex information on how to correct health record documentation, often in an urgent timeframe.

This role investigates the incorrect information and incidents assigned to them and performs the subsequent chart corrections. The Specialist I collaborates with clinicians and eCare teams as part of their daily work. The Specialist is part of an ongoing response team who responds to critical incidents during off hours to ensure that clinical errors are managed in a timely fashion.

Principal Duties and Responsibilities:

  • Investigates incorrect documentation issues identified within entity and enterprise clinical systems. Identifies root cause and resolves the issue to maintain a complete and correct legal health record for each patient. These may include issues such as: note retractions, moving data for visits booked on the wrong patient, correcting wrong information in the medical record, or multiple patients' information in one record.
  • Independently decides on the chart correction strategy for critical registration and documentation errors impacting patient safety. Leads clinicians, ancillary & clinical departments through the necessary actions that must be taken to correct errors.
  • Support end users at the elbow, over the phone and online. Meticulously track progress on issues and communicate with management and end users to allow all stakeholders to understand where their items are on the path to resolution.
  • Identify the root cause for problems and appropriately engage other teams to bring swift resolution.
  • Develop a deep understanding of how the Mass General Brigham Epic application is built and be able to explain to end users. Maintains in depth understanding of data flow between the EHR, various repositories and ancillary systems. Attain a general understanding of site level information systems both active and historical (legacy).
  • Responds quickly to wrong registrations on patients admitted or in the emergency department at any Mass General Brigham entity on a 24/7 basis. Directs multiple stakeholders (clinicians, support staff, IS) as to the appropriate actions to take to quickly resolve data quality issues and minimize the impact to patient safety using a rapid response methodology.
  • Communicate with end users throughout the chart correction process, utilizing e-mail, phone calls, in-basket messaging and Service Now updates. Ensures end users have understand process and what is required of them to resolve issues.
  • Participate in correction of large-scale system errors due to system conversions, build issues and clinical system bugs.
  • Guides patients through the amendment request process. Coordinates the request to amend the clinical information in a patient's medical record, in accordance with federal (HIPAA) guidelines. Coordinates with Mass General Brigham entities on multi-site corrections to facilitate a seamless (one patient, one record) patient experience. Makes addendums in the electronic record while communicating with all departments affected.
  • Provides feedback and makes recommendations to DH eCare HIM team for changes and enhancements related to eCare HIM/Identity functionality. Assists the DH eCare Identity team members in preparing detailed specifications and proposals for modifying the existing system when the solutions follow clear precedence. Works with the EMPI and PeC HIM teams to review, test and implement new functionality for the EHR.
  • Coordinates with EMPI Operations Team or Site Data Integrity specialists when a duplicate medical record or deactivate is identified or created during a chart correction.
  • Engage patients to verify clinical and demographic information to assist in record integrity corrections. Updates and validates patients' demographic information in eCare/Epic in compliance with department policies.
  • Performs other duties as needed.

Qualifications
Qualifications:
  • Associates degree in Health Information Management, Information Systems, Health Care Administration, or related healthcare field required. Bachelor's degree preferred.
  • 1+ years experience in a Health Care setting required, acute hospital experience preferred.
  • Demonstrated knowledge of Microsoft Office suite of applications, including MS Access.
  • Knowledge of electronic health record required. Epic knowledge preferred.
  • Bi-lingual strongly preferred

  • Strong PC skills required, knowledge of PC applications, e.g., Microsoft Office Suite, Microsoft PowerPoint; experience with Epic or other electronic health record or practice management systems preferred.

Working Conditions:
  • Participation in evening/night/weekend on-call rotation schedule is required. Rotation is 1 week amonth that is 24/7 on call.
  • Hospital work environment working conditions include possible exposure to diseases or infections and may require safety gear (PPE) such as gloves and mask.
  • Normal office working conditions. The noise level in the work environment is quiet to moderate.
  • While performing the duties of this job, the employee is frequently required to sit; talk; or hear; use hands to finger; handle; or feel; reach with hands and arms. The employee is occasionally required to stand; walk; and stoop; kneel; or crouch. The employee must frequently lift and/or move up to 5 pounds and occasionally lift and/or move up to 20 pounds.
