Emergency Room Social Worker, LCSW - Part Time

Schedule: part-time 16 hours; Weekends only (Saturdays and Sundays).

The clinical social worker will provide psychosocial assessments and interventions for patients and their families in the Emergency Department. The social worker must be able to intervene using best clinical practice regarding a medical crisis, trauma, concern for abuse/neglect and filing mandated reports when indicated, assess for and provide treatment planning for substance use disorders (familiarity with treatment modalities and resources), assess for and provide problem solving regarding barriers to discharge from the ED as well as barriers to inpatient admission when indicated, and collaborate with community resources and agencies to expedite patient care needs. All interventions will be documented in a patient's electronic health record.

PRINCIPLE DUTIES AND RESPONSIBILITIES:

  • Will assist with urgent weekend coverage on inpatient floors including ICU as needed.
  • As member of interdisciplinary team, will collaborate with MD, RN, RNCM, and all other disciplines involved with the patients' care.
  • Will participate in daily huddles regarding patients' medical and psychosocial needs and will remain available to ED staff for social work consultation as indicated.
  • The ED Social worker will provide psychosocial social assessments and clinical interventions for patients in the emergency department which may involve the following:
    • Medical crisis (death, new diagnosis, sudden medical change requiring help reaching HCP agent/emergency contact and then supporting patient and/or surrogate decision maker, sudden need for surgery, coping with any of these issues, assisting with family meetings as needs arise, etc)
    • Traumatic event (MVA, fall, assault, intimate partner violence, etc)
    • Concern for abuse or neglect of elder, disabled person, child, or due to possible nursing facility negligence. Filing mandated reports when indicated for EPS, DCF, DPPC, or DPH.
    • Homelessness (assessment, resources, problem solving)
    • Substance Use assessment, resources, and potential assistance and referral for treatment if indicated and patient is motivated. Will collaborate on SUDE with ED team.
    • Behavioral concerns not requiring psychiatric involvement (support and help containing affect or coping needs of patients or their family members)
    • Barriers to discharge from the ED in collaboration with RNCM (working on eliminating any barriers that can be addressed quickly in the ED such as applying for The Ride due to medical necessity, etc)
    • Barriers to inpatient admission when the ED providers determine the patient needs to be admitted for medical necessity and the patient is reluctant or refusing
    • Code Blue response as clinical member of Code team in all areas that are NOT on the inpatient floors as those will be responded to by the inpatient social worker for that unit unless on a weekend where the ED social worker (per diem) covers both inpatient and ED areas.
    • Psychosocial assessments for patients with frequent use of ED vs outpatient care or frequent use of ED with no medical issues identified to help determine other course(s) of care.
    • Participation on appropriate hospital wide committees as determined by Social Work Manager such as Acute Care Plan or Safety Committee.
    • Participation in daily interdisciplinary ED huddles/rounds regarding patient care needs.
    • Supportive check-ins and/or meetings with ED staff for debriefing of difficult or compelling cases.
    • Other duties as determined by Social Work Manager.

Qualifications
  • Master's in Social Work
  • Licensed (preferred) or license eligible: LCSW, LICSW
