The Duke Heart Center Electrophysiology (EP) Device Nurse/VT Coordinator is responsible and accountable for patient and staff education around implantable devices and procedures, device interrogation and documentation. Ensuring appropriate follow-up for patients with implanted rhythm management cardiac devices (pacemakers, defibrillators, implantable loop recorders) and post ablation procedures for ventricular arrhythmias.. The incumbent functions are as clinical nurse, educator, investigator, and consultant. Clinical judgments are based on a synthesis of data from current and emerging research, concepts, principles, theories, and outcomes to solve complex problems. The Cardiology EP Device Nurse/VT Coordinator provides direct patient care services and education to patients in the post procedural, and monitoring phase of rhythm management and cardiac device implantation and follow-up primarily at Duke University Hospital. The Cardiology EP Device Nurse/VT Coordinator will work primarily with cardiology patients who are being evaluated for cardiac rhythm management, devices in post implant care including setting up appropriate follow-up at Duke University Hospital (DUH) as well as patients undergoing specific EP rhythm management procedures. The Cardiology EP Device Nurse performs post procedural screenings, educates patients and their families on device-specific care and ongoing monitoring requirements and procedures, and coordinates and performs implantable device interrogation, programming, and documentation in the electronic medical record under the supervision of the Electrophysiology Cardiologist rounding on the inpatient service. The Cardiology EP Device Nurse/VT Coordinator partners with the inpatient and outpatient clinical care teams to coordinate and expedite transitions of care including admissions and discharges for patients with implanted devices/VT ablations and may provide direct patient care services, such as wound assessment and dressing changes as well as follow-up of patients post arrhythmia treatment procedures, such as ablations. The

The Cardiology ED Device Nurse/VT Coordinator will assist in the development and implementation of general policies and procedures designed to effect efficiency of operations.

Position Requirements

Education:

Work requires graduation from an accredited nursing program with a minimal requirement of a BSN and at least one year of experience

Licensure:

RN: Current licensure or eligible for licensure as a Registered Nurse in the state of North Carolina.

Experience:

Strongly prefer candidate with at least three years of experience as an RN in an inpatient cardiology setting, including care of patient with implantable rhythm management cardiac devices and competency in basic rhythm interpretation. Expertise and clinical competency in implantable cardiac device interrogation and programming required.

Certifications:

Must have American Heart Association BLS certification

International Board of Heart Rhythm Examiners (IBHRE) device certification preferred

Hours

Primary hours will be Monday thru Friday dayshift. Schedule may be changed by administrative or clinical leadership to meet departmental needs.

    • Able to navigate the Hospital and DUHS to provide clinical expertise to specific patient populations
    • Able to withstand prolonged standing and walking with the ability to move or lift at least fifty pounds
    • Able to remain focused and organized
    • Able to work collaboratively with all levels of personnel

    Responsible to

    Reports directly to the Reports directly to Nurse manager EP lab with clinical responsibility to the inpatient rounding Cardiac Electrophysiologist. In addition, s/he will be accountable to the Division of Cardiology and the Duke Heart Center for clinical outcomes, defined quality indicators and to the standards set forth by the Duke Heart Center, the Division of Cardiology, and the Chief of the Section of Cardiac Electrophysiology.

    Process of Evaluation

    Annual and midyear evaluations will be conducted by the Nurse Manager or their delegate with input from the Chief of the Section of Cardiac Electrophysiology and multidisciplinary team members who work with the device/VT nurse.

    Specific Job Responsibilities and Standards

    Clinical ExpertThe Cardiology EP Device Nurse is responsible for providing/coordinating direct clinical care to cardiology patients on the Cardiac Electrophysiology service and other inpatient services, Emergency Department, or those requiring inpatient and hospital based outpatient procedures performed at Duke Hospital pre and post implant of a cardiac device. The Device Nurse is also responsible for maintaining the practice consistent with the rules and regulations of the North Carolina Board of Nursing and Duke Hospital Nursing Practice Standards. Standards
    • The Cardiac EP Device Nurse will evaluate new patients, consults and established patients, provide education, arrange or personally perform device interrogations and reprogramming of device under direction of EP attending, maintain documentation and reporting around implantable device interrogations in partnership with the multidisciplinary team
    • The Cardiology EP Device/VT Nurse provides direct patient care services and education to patients in the post procedural, and monitoring phase of rhythm management and cardiac device implantation and VT ablations and follow-up primarily at Duke University Hospital.
    • Work closely with the inpatient rounding teams including attending, advanced practice providers (APPs), residents and Fellows to provide optimal support and care to patients with implantable cardiac devices
    • Assist in the training of new employees, students and Fellow trainees, to the limits of competence, and actively participate in educational conferences
    • Actively participate in the discharge process for all assigned admitted/procedural patients, including but not limited to, providing individualized patient and family education with discharge instructions including documentation, coordinating follow-up appointments and monitoring including remote monitoring, assuring documentation of device programming and interrogations, education, and patient wound care instructions
    • Will provide Pre/Post Procedural follow up calls to patients for arrhythmia and device procedure patients
    • Will develop standard work and algorithms around inpatient device interrogations and post ablation follow-up

    Educator:

    • Assess educational needs of the patient-care staff and provide one-on-one as well as formal education as needed
    • Assess educational needs of the patients / families and provide culturally-sensitive patient/family education interventions as indicated. Use non-technical language when educating patients and families and document all education in patient's electronic medical record
    • Develop and promote use of educational materials, including pamphlets, videos, and web-based content
    • Implement and evaluate appropriate educational programs for DUHS patient care staff and other healthcare providers as appropriate, including speaking at conferences such as Heart Rhythm Society (HRS)
    • Role model and precept healthcare learners through contractual agreements

    Research Utilization and Investigation:
    • Utilize current research and evidence-based decision-making in all clinical practice.
    • Incorporate clinical research findings in the development and implementation of standards of care
    • Support IRB approved clinical research/trials though a variety of activities including but not limited to: obtaining consent; ordering diagnostic tests; recording laboratory & diagnostic results; administering therapeutic interventions; and reporting patient outcomes including toxicities or adverse events
    • Participate in interdisciplinary quality improvement and/or research activities.
    • Participate in measuring and documenting outcomes

    Consultant:

      Provide expertise and resource information to other healthcare providers, patients and families, and the community at large regarding cardiac implantable electronic devices and heart rhythm disordersDevelop, implement and evaluate standards of care/practice guidelines/policies and procedures/care maps/protocols within area of specialization
    • Develop programs and projects in areas of expertise to facilitate the attainment of the goals and objectives of the Duke University Hospital and Duke University Health System

    Working conditions

    The work environment involves risk discomforts that are typical of an acute care setting to include working with patients under isolation and universal precautions. It requires safety precautions, ongoing education, and health risk monitoring. Contact with specimens, chemicals, sharps, radiation, technical equipment and fumes are possible. Employees are required to wear personal protective attire according to hospital policies and OSHA guidelines. The work activity and patient acuity levels can create a stressful atmosphere.
