Essentials:
  • TCU case management
    Discharge planning
    MDS assessments
    Care planning
    Providing resources, and referrals to residents/families/staff members
    Social Worker Requirements:
  • Self-motivated to serve residents
    Possess strong organizational/communication skills
    Able to multi-task well
    Be team player oriented
    1 year of experience with LTC preferred .
  • Coordinates resident admissions
  • Coordinates and leads transfer and discharge team
  • Completes assessments of residents as necessary for Care Planning, MDS, RAPS, PPS and other required documents
  • Develops and promotes the department through marketing efforts throughout the professional and social community

Job Requirements:

Required Qualifications:
  • Bachelor's degree in Social Work or related degree in social sciences; Licensed in the state of employment.
  • Minimum of one-year of Social Work experience, in a healthcare setting; knowledge of home health care and public health settings; familiarity with Federal and State regulations and requirements for home health care.
  • Twenty-one (21) years of age, meet the MVR organization standards, and have a valid state driver's license.
  • Ability to read, speak and write fluently in English.
