JOB SUMMARY:

Intake:

The Registered Nurse is responsible, in coordination with the designated Unity Health provider and the HHAH clinical social worker for timely pre-admission clinical review of potential patients, to include admission and re-admission screening tools, MARs and TARs, clinical and rehab notes, labs, and other necessary information. The RN will perform baseline and ongoing assessments of potential patients, and gather all pertinent information regarding referrals as defined by VOACC and the Hope Has a Home (HHAH) Medical Respite Program policy. Understands medical diagnoses, prognosis and clinical indicators in order to make an appropriate decision regarding admission to the Hope Has a Home Medical Respite program.

On-going care:

The HHAH Registered Nurse will be responsible for coordinating with the Unity provider the on-going medical care of program residents until their discharge to a less supervised setting. This includes on-gong evaluation of both the progress of medical treatment and the development of the self-care and self-management skills needed to manage medical conditions independently. The HHAH RN will participate in the decision to discharge to a more independent setting.

Supervision:

The HHAH Registered Nurse will supervise the HHAH LPNs.

REQUIREMENTS:

  • Successful candidates must be a graduate of an accredited school or program of nursing, BSN preferred.
  • Possess a current, valid, unrestricted Registered Nurse licensure by the District of Columbia Board of Nursing or must be willing to apply for DC licensure within 90 days of hire.
  • Experience working in a multi-cultural human services non-profit organization.
  • Ability to learn management systems (i.e. Paycom, Health Stream), creating learning management system curriculum as well as conducting and facilitating virtual and in-person trainings.
  • Experience driving and implementing evidence-based practices (EBPs) and aligning EBPs to diverse integrated service lines (i.e. Intellectual/Developmental Disabilities, Homeless Services, Justice-Involved, Behavioral Health, Mental Health, Substance Use Disorders, Primary Care, Veterans, Supportive Housing).
  • Experience with continuous quality improvement systems and development of quality plans for areas of residential and non-residential settings in the behavioral health and homeless services for veterans and family populations.
  • Experience with CARF, ACA and/or other accreditation bodies (i.e. JCAHO, NCQA).
  • Experience with diversity, equity and inclusion initiatives and implementation.
  • Experience working with and deploying electronic health records (i.e. Foothold/Awards).
  • Experience with and knowledge of diverse funding streams (i.e. fee-for-service, cost reimbursed, pay for performance, Medicaid billing, MCO contracts, etc.).
  • Ability to effectively collaborate and facilitate change, interfacing with all levels of the organization while managing multiple projects over multiple states.
  • Strong problem solving and execution skills.
  • Proven change management skills with excellent verbal and written communication skills.
  • Experience working with state health plans, MCOs, ACOs, HEDIS measures a plus.
  • Bilingual in any language a plus.

RESPONSIBILITIES

Majority of the tasks performed by an RN are regardless of their workplace environment, whereas some solely depend on the clinical settings. Here are some of the prominent RN duties and responsibilities:

  • Must have thorough understanding of the process of clinical rehabilitation, expert physical assessment and patient education skills.
  • Performs on site, record and telephone preadmission screening/clinical assessment to determine appropriateness of referral and medical stability for inpatient acute rehabilitation program.
  • Completes assessment/evaluation within 24 hours or in one working day of the referral (documents exceptions within the electronic medical record).
  • Reviews clinical information in patients medical record and performs physical assessment of the patient to verify/confirm information in medical record.
  • Interviews referring healthcare team members, patient, other stakeholders and prepares a summary report at time of initial on-site evaluation.
  • Provides preliminary recommendation to referral source regarding appropriate level of care of potential patients.
  • Reviews clinical data/assessment with Peer Pathways Healthcare Team member (Social Worker) and/or Dedicated House Medical Provider (MD/NP) for determination of medical stability and appropriateness for admission and notifies referral source of decision.
  • Educates patient, families or significant others, referral sources, hospital personnel, external payers and case managers regarding Medical Respite program services and benefits, admission criteria, bed availability, day treatment programs, outpatient centers/programs, and basic knowledge of third-party reimbursement.
  • Maintains marketing plans for each assigned hospital. Maintains a thorough knowledge of HHAH facilities, products, and services. Develops and maintains ongoing relationships with referral sources. Prepares/submits marketing reports and all necessary data for accurate maintenance of statistics.
  • Provides feedback on admitted patients progress at HHAH to referral sources as requested.
  • Provides recommendation for alternative level of care when patient needs cannot be met at HHAH Medical Respite Facility.
  • Review and submits preliminary treatment plan, estimated length of stay and clinical goals process as requested.
  • Communicates verbally with appropriate HHAH/ VOACC, Unity, and Peer Pathways staff regarding special needs e.g. equipment, social issues, etc.
  • Help to coordinate patient referrals and take part in the training and orientation of new employees, and interns as needed.
  • Call pharmacies for ordering prescriptions and store supplies as needed including other duties not listed.
  • Review appropriate paperwork for documentation of clinical services and clinical assessments.
  • Enter clinical data in computer systems (Footholds/ Awards, etc.).
  • Keep the health records updated.
  • Perform other duties as assigned.
Quality Assurance and Compliance:
  • Expand and support organizational competence, credentialing, specializations and accreditation in relevant areas such as, but not to: Healthcare integration, Multicultural Competence/Care, Trauma Informed Care, etc.
  • Participates in any HHAH accreditation processes as requested.
  • Guide the development and implementation of standards supporting an integrated care delivery system; data management to support decision-making; and manage health
information quality and outcomes (i.e. electronic health record choice for organization).
  • Must be compliant with the design and monitoring of the organizations continuous quality improvement and assurance programs.
  • Ensures and demonstrates compliance with federal, state, local, and accreditation bodies regulations and standards, as well as VOACC policies and procedures at all times
