SUMMARY: The Advanced Practice Nurse carries a caseload of patients, caring for them throughout the health care continuum. The scope of practice is defined by a collaborative agreement with an appropriate physician. The Nurse Practitioner also serves as a Case Manager for patients who require his/her clinical expertise to coordinate care. The Nurse Practitioner has a leadership role in nursing, providing education and consultation in areas of expertise.

ESSENTIAL FUNCTIONS:

Direct Patient Services 80%; Support Services - 20%

  • Utilizes advanced knowledge, skills and expertise in pediatrics to implement an APN role for an identified population of patients, including primary care, health promotion, prevention of disease and management of chronic disease. Performs delegated medical functions as described in written protocols and/or a collaborative agreement which has been approved by medical and nursing administration.


  • Systematically assesses the physical, psychosocial and developmental needs of patients and families through the use of a history and physical exam and other assessments as appropriate. Develops a plan of care for each patient/family based on assessment and documents according to policy.


  • Demonstrates advanced nursing clinical judgment and competence in assessment, planning, implementation and evaluation of identified patients. Monitors clinical outcomes to improve the quality of care for patients. Evaluates and modifies the plan of care as needed.


  • Facilitates collaborative practice with the multidisciplinary team to provide a comprehensive, coordinated needs assessment and treatment plan. Communicates the plan to patient, family, and other care providers, both within the Hospital and with appropriate community agencies to provide continuity of care.


  • Integrates the role of teacher into clinical practice to provide patient education. Assesses learning needs, provides for education, and evaluates learning achieved. Assists the staff in providing patient and family education.

  • Job Requirements:

    QUALIFICATIONS:

    Education:

    Master's Degree in Nursing in appropriate specialty

    Related Work Experience:

    Five years of clinical experience in pediatrics

    Licensure/Certification:

    Current Licensure as an RN and APN (or eligible for APN license) in the State of Illinois; Current certification in appropriate specialty or eligible for certification exam

    Specialized Knowledge and Skills:

    Excellent verbal and written communication skills, excellent interpersonal skills, demonstrated knowledge and expertise in clinical management in area of specialty, family education, and case management, active membership in professional nursing organization(s)

    CPR required:

    Only certifications issued by the American Heart Association or the American Red Cross will be accepted.
