Benchmark Human Services has grown to become one of the most respected organizations in the country in the areas of intellectual and developmental disabilities (IDD) and behavioral health. We work with people at home, at work, and in the community, through residential, employment, crisis response, behavioral, recovery, children's services and day services.

Benchmark is excited to begin serving children, ages birth to three through our Early Intervention program in Colorado. We are seeking Physical Therapists in Boulder, Broomfield, Fort Collins, and Greeley.

Responsibilities

  • Conducts evaluations and assessments
  • Participates in Individualized Family Service Plan (IFSP) development and implementation
  • Comply with all Federal and State regulations, including those outlined in the Colorado Early Intervention Regulations
  • Administer and provide evaluation activities for children referred with a suspected developmental delay
  • Develop rapport with children, families, and team members to promote a healthy learning environment
  • Participate in a multidisciplinary evaluation
  • Participate in state-sponsored training, meetings and learning opportunities as appropriate
  • Provide families with information, skills and support related to enhancing their child's development
  • Enter evaluation results into the EI Data System Provider Portal within the required timeline
  • Comply with all standards to assure the health and safety of all individuals
  • Ability and Willingness to travel throughout designated region to provide services
Qualifications
  • Bachelor's, Master's, or Doctorate Degrees and CO State Licensure in Physical Therapy
  • Valid Colorado Driver's License
  • Valid Auto Insurance
  • Preferable One Year of Experience Working with Children in Early Intervention
  • Ability and Willingness to travel throughout designated region to provide services
