Summary

Provide direct patient care for an assigned group of patients and also responsible for the overall delivery of care for a specific shift on the unit. Coordinate patient assignments and delegate responsibility to professional and non-professional staff members to ensure the completion of safe and comprehensive patient care and optimum patient satisfaction. Act as role model and resource for staff members.

Key Responsibilities

  • Nursing assessment is completed accurately and timely in accordance with practice standards.
  • Plan of care is developed and implemented that reflects the needs and goals of the patient.
  • Nursing care is provided, evaluated and modified to achieve goals and objectives.
  • Documentation is completed according to department policies and procedures.
  • Medications are administered safely and according to professional standards of practice and nursing policies and procedures.
  • Nursing care activities are coordinated in collaboration with other care team members to facilitate achievement of patient goals.
  • Patient education needs are assessed, and education provided related to the disease process, underlying disability, medications, and preparation for discharge to help patients and families develop self care skills necessary to move toward wellness.
  • Mentoring and teaching is provided for healthcare team members regarding nursing rehabilitation practices.
  • Team conferences and meetings are actively participated in to achieve stated goals.
  • Patient assignments are made to meet patient care needs, continuity and availability or resources.
  • Nursing staff are assisted with communication issues that may exist between physicians, interdepartmental team members, consultations or peers to achieve resolution.
  • Open communications are provided to Nurse Manager on major issues, adverse events, risk management and/or deteriorating patient condition.
  • Direct and indirect support is provided for nursing staff as needed.
  • Nursing staff is assigned for scheduled breaks and meetings while ensuring patient needs are met.
  • Code/emergency care check is assigned daily per established policy.
  • Preceptors are assisted in the orientation of new employees to achieve performance goals.
  • Professional quality improvement activities are participated in to achieve unit goals.
  • Staff adherence to regulatory guidelines is evaluated and staff is supported to make any necessary adjustments in compliance.
  • Additional department, organization, or network activities are completed per established objectives.

Qualifications
Qualifications and Experience

Education/Degree

  • Bachelor of Science in Nursing preferred.

Experience

  • Minimum of 1 year clinical experience, post grad.

Licensure

  • Current licensure from the Massachusetts Board of Registration to practice professional nursing.

Skills

Effective verbal, electronic and written communications skills using the English language.

  • Computer proficiency required. Microsoft office applications preferred with ability to learn new software.
