Join our team of competent and compassionate professionals as a Counselor-Case Manager in the Connectionsprogram at Behavioral Health Network!

Your experience, knowledge and great skills will support the Connections program with the following:

  • Performing thorough individual assessments of individuals served.
  • Developing individual treatment plans using knowledge of the range of addiction, dual diagnosis, methadone issues, and aftercare planning, as well as the special needs, strengths and weaknesses of individuals served.
  • Assessing the outcome of treatment planning, adjusting treatment approaches accordingly, and documenting according to established expectations.
  • Providing individual and group counseling sessions to maximize quality and retention in treatment.
  • Educating individuals & groups regarding alcohol and drug abuse, harm reduction, mental illness and available community and self-help resources.
  • Coordinating referrals for ongoing aftercare treatment with agencies and services and facilitating individual's responsibility and participation in aftercare planning.
  • Assisting individuals served toward individual needs regarding resolution of legal issues, homelessness, medication compliance, economic difficulties, employment and continuing care needs.
  • Providing transportation to individuals served to appointments related to aftercare for medical reasons, as needed.
  • Providing care with respect, sensitivity and in a manner that preserves and protects client autonomy, dignity and rights.

Who are we?

The Connections program offers residential substance use disorder treatment for adults 18 years of age and older as an alternative to incarceration. The multidisciplinary team at Connections partners with each program participant to create a strengths-based, goal-oriented treatment plan to provide a road map to meet individuals' self-identified goals for improved physical and behavioral health, stable housing, educational or vocational pursuits, parenting needs, and recovery supports. The goal of treatment is to assist individuals in navigating the challenges of early recovery and help them to develop the skills needed to reduce risk of relapse or further engagement with the justice system.

What are we looking for in a Counselor-Case Manager?
  • High school diploma or GED equivalent (required); Bachelor's Degree in related field (preferred).
  • Credentialed as CAC, CADC, LADC-Assistant, LADC II (preferred).
  • At least two years of experience in substance abuse or behavioral health counseling (required).
  • Ability to understand, empathize and work with an addicted, dually diagnosed and/or methadone maintained population.
  • Demonstrated ability to approach individuals served in a sensitive and therapeutic manner.
  • Ability to assess the range of addiction, dual diagnosis and methadone issues.
  • Ability to recognize the special needs of individuals served and adjusting treatment approaches accordingly.
  • Experience with providing case management and/or counseling services to patients in a treatment setting (preferred).
  • Valid driver's license (required).
  • Vehicle for use during all working hours should be registered and inspected in accordance with 540 CMR 4.00 (required).
