POSITION SUMMARY:

The Population Health Management (PHM) Analyst supports Mercy Care's overarching Strategic Goal to be a high quality and effective partner in population management. Working within the Quality Department and interfacing with departments throughout the organization, the PHM Analyst helps Mercy Care plan, carry out, and coordinate activities aimed at improving the health of Atlanta's homeless and underserved populations. The PHM Analyst is responsible for research and analysis that will guide the development of organizational initiatives directed towards improving health outcomes, addressing social determinants of health, and decreasing heath care utilization costs among targeted population(s).

The Population Health Management Analyst will gather health care data from a variety of environments, such as: public health data, electronic medical records, client feedback, and billing/claims data as available, and will segment and stratify as appropriate to describe various aspects of the population, e.g. high risk. The PHM Analyst will also collect and evaluate evidence from best practice, priorities of the target community, and strategies of local government and partner agencies, and present analysis and findings to Mercy Care and community stakeholders. He/she will develop improved methods of forecasting upcoming opportunities, will provide research and data to guide implementation of initiatives, and will participate in program evaluation and outcomes management. The efforts of the PHM Analyst will position the organization to identify and respond to specific community needs and upcoming programmatic opportunities.

Specific duties may include, but are not limited to: participating in needs assessments, holding focus groups, meeting with community partners, collecting and analyzing data, stratifying and segmenting data to identify high risk population, presenting findings in meaningful ways to various audiences, recommending interventions, and writing sections of proposals and concept papers.

EDUCATION REQUIREMENTS: Graduate level credentials in a public health related discipline including, but not limited to, population health, epidemiology, and the translation of data into the design and implementation of organizational strategic plan initiatives. Master's degree in Public Health or Health Administration strongly preferred.

EXPERIENCE REQUIREMENTS:

Clinical background or exposure is essential. Experience in a public health setting, community health center, and/or FQHC preferred. Proficiency with finding and evaluating scientific research and navigating automated information management systems. Experience with quality management and formulation of outcome measurements. Familiarity with conducting needs assessments and focus groups. Proficient in manipulating large excel databases. Experience in UDS reporting.

JOB KNOWLEDGE AND SKILLS:

Understanding of population management, health revenue and reimbursement, and determinates of health. Exceptional analytical, organizational, and written and oral communication skills. Attention to detail. Proficiency with collecting, reviewing, analyzing, segmenting, and presenting data. Comfort using data to drive program design. Ability to engage, network with and collaborate with community partners. Able to work independently. Preference given to candidates with health modeling skillset.
