JOB SUMMARY

This position will report to the Medical Director and is responsible for acquiring and demonstrating clinical expertise and knowledge of mental health sub-specialty programs to improve the quality of consumer care, the use of resources, and to facilitate compliance with internal and external requirements and standards; acquiring and demonstrating strength in the areas of interpersonal relations, critical thinking, problem solving, and conflict resolution; collaborating with other mental health clinicians, the Interdisciplinary Treatment Team and other clinical and administrative staff as needed to ensure patient care needs are met. The position is responsible for assisting the Triage Clinic with medical support for patients who present with an identified need.

RESPONSIBILITIES

  • Communicate with medical/care providers.
  • Assist the consumer in the development of interpersonal and community coping skills.
  • Assist the client in the development of relapse and symptom management strategies and plans.
  • Participate in the preparation of the Individualized Service Plan with specific, measurable, behavioral objectives, and action plans.
  • Provide and coordinate referrals to make sure that the consumer receives the requested assistance.
  • Conducts visits to the consumers job site, home, hospital room and other natural settings.
  • Be available, on-call as scheduled, for nursing consultation and/or treatment of minor illnesses or injuries and render nursing assistance in medical emergencies.
  • Prepare, if required, Medication History at admission and perform periodic re-evaluation, monthly medication administration review and quarterly systems review.
  • Coordinate medical appointments.
  • Assess, plan, and follow up on medical issues noted with clients to include location of needed medical resources in the community, as appropriate.
  • Performs other duties as assigned.

EXPECTATION
  • Participate a physician member of a multi-disciplinary patient care team.
  • Complete all billable notes within 24 hours of service rendered.
  • Provide consultation to behavioral health team as needed.
  • Work effectively with departmental staff, private physicians, clients and others contracted in the course of work.
  • Community collaboration with key health stakeholders, other divisions of the Department, and other County departments.

EFFECT ON END RESULT
  • Maintain effective relationships with other County Departments, County policy makers and community health stakeholders
  • Conduct in-service training programs as needed.
  • Creates and maintains an environment of care for persons served and becomes a value member of the community.

PHYSICAL REQUIREMENTS

  • The ability to safely operate a motor vehicle to transport oneself, consumers, and program supplies as necessary.
  • The physical ability to travel to assigned locations, stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, hear and perform basic and light home maintenance activities, and operate office equipment.
  • Move and lift light objects up to 30 pounds such as mail, supplies, files, and equipment.
  • Operating office equipment requiring continuous or repetitive hand/arm movements.
  • The ability to remain in a sitting position for extended periods of time.

REQUIREMENTS
  • Current Licensed as a Registered Nurse in the State of Maryland.
  • Minimum two (2) to four (4) years of experience practicing primary care within a community health center (FQHC) or Clinic.
  • Graduate of an accredited Nursing Program and certified by the American Nurses Credentialing Center and/or a degree in nursing or related field a plus. Degrees from outside of the United States must be reviewed and accepted by the World Educational Services and validated.
  • Must have CPR/First Aide certification.
  • Trained in cultural competency and working with diverse populations.
  • Must have experience with electronic medical records systems foothold/awards
  • Excellent computer skills in Microsoft Office Suites.
  • Excellent written and verbal communication skills.
  • Bilingual in any language a plus.
