Position Summary: LifeMoves is currently seeking a family-centered Bilingual Community Case Manager to join our Upstream program, which serves and collaborates with referred families and youth in Redwood City, CA through community case management. Responsibilities include providing outreach in a school and community setting; intensive case management to households who are referred by Kennedy Middle School (in RWC) and are at-risk of or experiencing homelessness; completing screening assessments and case plans; making appropriate referrals; and collaborating with the team to continue the development and implementation of the Upstream program. The successful candidates will have the ability to represent LifeMoves in a diplomatic and professional manner with partners, funders, and other city/county entities and must be bilingual in Spanish to serve our primary Spanish speaking families.
